I looked at some keyword suggestions using software that lists the frequency a keyword phrase occurs in searches.

Two words of a phrase are repeated in several suggestions with a third word added to complete the phrase.

How many times can one repeat a portion of the phrase before running into spam issues?

I think there is a limit to the number of relevant keywords that you can use before you start getting a penalty.

I can see where some repetition is expected because the number of terms that can be used is huge.  I suppose the repetition in the tags should be considered when building content as well.

Well, it's like this: you want to repeat search terms relevant to your site, but, you don't want to oversaturate.  The idea is your content should be relevant to the keywords, with still being readable and useful (which 50% keyword saturation obviously can't be).

So instead of repeating in the tags so much, some of those repeated terms should be found in the site itself.  That makes sense and helps pad your content if you feel uninspired!

Search engines don't use keyword density much anymore to determine rankings.  They look for overall content that relates to the topic.  You should use the main keyword phrases you'd like to target in you Title, Meta tags, alt tags, body content and header tags. If you use your keywords once or twice in each, Google and other search engines will get the idea of what you're trying to target.

Are the lengths of visits included in the ranking process?  With the advent of video embeds as sales tools, will some of the algorithms be changed?

Slingshot is correct.  You need to spread your keywords and phrases throughout your website content.  Just make sure that you don't use keywords that are too similar.  I know one company that has 3 keyword phrases.

Company name new york, company name ny, company name nyc

These are the only keyword/phrases this particular company is using right now, which limits the results they will get on any search.  Mainly, because not everybody is going to know the company name.
